Tomatis therapy is based on the distinction between the process of hearing and the function of listening. In children or adults, because of various injuries or damage, a disorder may occur in which a person hears but does not listen, meaning the brain does not respond to certain sound stimuli. The patient may pay attention to extraneous sounds but not respond when spoken to. Therapy sessions are conducted with a special device, a model of the human ear. It is a kind of training of the ear, on which, according to the method's proponents, the functioning of the patient's brain ultimately depends.

Today there are many offshoots of Tomatis therapy, and different centers work with different programs, equipment and levels of specialist training. When choosing a center, it is worth asking which programs are used, how the course is structured and what training the specialists have.
The basis of the therapy is always as follows: the first course lasts 15 days. The second course should include a recording of the mother's voice (intrauterine listening). The third course consists of "sound birth." The fourth includes speech phases, and so on.
