Trochanteritis is a degenerative inflammation of the tendons and surrounding tissues.

Trochanteritis and the "Stripe" Syndrome: What Does the Condition Indicate and How to Deal With It?

Trochanteritis is inflammation or irritation of the tendons and the bursa around the joint in the area of the greater trochanter, the bony prominence on the side of the thigh, which causes pain on the outer surface of the hip joint and thigh. This pain is often called "stripe" pain because it runs along the side of the leg like the stripe on trousers.

In brief:

At CSM Clinic in Kyiv, appointments are conducted by a neurologist; the treatment methods include shockwave therapy.

The "stripe syndrome" points to compression of the nerve at the fifth lumbar vertebra as a result of an intervertebral hernia, inflammation, or muscle spasm. This condition often radiates into the leg and can make it impossible to move around independently. Shockwave therapy (SWT) is now available, which may help normalize muscle function and relieve pain.

Causes of Pain in the Hip Joint

The "stripe syndrome" may point to the development of trochanteritis, a degenerative inflammation of the tendons and adjacent tissues in which their attachment to the thigh bone (the trochanter) is impaired.

If the "stripe syndrome" results from trochanteritis of the hip joint, the symptoms will be as follows:

Trochanteritis can be caused by infection (as a complication of chronic caries, sinusitis, or tonsillitis), by a tuberculous focus in the area of the hip bone, or can be aseptic (due to injury, changes in body weight, or high loads). The main cause of the disease is reduced elasticity of the ligaments.

Treatment of Trochanteritis and the "Stripe Syndrome"

Trochanteritis is generally easier to treat than other conditions of the pelvis and hip area. Depending on the nature of the disease, doctors adjust the course of medication. However, it is also important to eliminate the "stripe syndrome" and prevent recurrence. Shockwave therapy may help with this.

CSM Clinic performs procedures aimed at relieving pain, relaxing the muscles that compress the nerve, and strengthening the elasticity of the ligaments in the area of the hip joint and lower back. As a result, the pain that restricts the hip may subside, metabolic processes normalize, and the muscles, tendons, and ligaments are strengthened.

Specialists at our center perform shockwave therapy using modern equipment. A gel is applied to the skin of the thigh, and then a device with a special applicator is placed on it. Shockwaves of a set frequency and strength pass through it. The whole procedure lasts 15-20 minutes. Such sessions are repeated every 3-5 days over a month. After the course, the patient may feel improvement: the external signs of the disease may lessen and the pain may ease. Recovery continues over the following months.

Frequently Asked Questions

Can trochanteritis be treated at home?

You can ease the condition at home, but the diagnosis must be confirmed by a doctor. Rest for the hip, avoiding sleeping on the painful side and prolonged strain, and a cold compress in the first days may help. It is better to choose painkillers together with a doctor. If the pain does not ease within 1–2 weeks, an examination is needed, because a pinched nerve and a joint problem can hurt in a similar way.

What is lateral thigh ("stripe") pain in the leg and what causes it?

Stripe pain runs along the outer surface of the thigh, sometimes down to the knee and shin, where the stripe runs on trousers. It is most often caused by trochanteritis or a pinched L5 nerve due to a herniated intervertebral disc, and less often by a hip joint disease. To tell the causes apart, the doctor examines the spine and the joint and, if needed, orders an MRI or ultrasound.

Which doctor treats trochanteritis?

Usually a neurologist or an orthopedic trauma specialist. It is worth seeing a neurologist if the pain radiates to the leg or lower back and a pinched nerve is suspected; an orthopedist if the joint itself hurts. A rheumatologist is needed when a systemic inflammatory disease is suspected. At CSM Clinic, the initial examination is performed by a neurologist.

How long does treatment for trochanteritis take?

A course of shockwave therapy at CSM Clinic lasts about a month: sessions of 15–20 minutes every 3–5 days. Acute pain usually subsides within a few weeks, while full recovery of the tendons takes longer. The timing depends on the cause and on whether the triggering factor, such as overload, has been eliminated.

Why does the side of my thigh hurt at night when I lie on my side?

When you lie on your side, the greater trochanter presses against the mattress, and inflamed tendons and bursa react to the pressure with pain. Sleeping on the healthy side with a pillow between the knees, or on your back, may help. If the pain wakes you every night and does not subside for a long time, book an appointment with a doctor: other diseases can also show up this way.

When should pain on the side of the thigh not be tolerated?

Do not delay a visit if the pain appeared after a fall or blow, if there is fever, redness, or swelling of the thigh, if you cannot put weight on the leg, or if the pain is unbearable and increasing. A fracture, infection, or other serious conditions may present this way rather than trochanteritis. Also tell your doctor about pain that lasts more than two weeks.
