State-Funded Rehabilitation for Children with Disabilities at CSM (Resolution No. 309)

Under Resolution No. 309, to receive free rehabilitation for a child with a disability in Kyiv at CSM Clinic (from the List of rehabilitation institutions and health care facilities that carry out rehabilitation of children with disabilities), the child's representative must submit an application to the local authority. In it, the applicant justifies the need to refer the child to a rehabilitation center.

What You Need to Do to Get Free Rehabilitation for Your Child at CSM Clinic in Kyiv

1. Submit an application in the established form to the Department of Labor and Social Protection of the Population (hereinafter DLSP) at the child's place of residence or registration.

You can download the application form on our website or draw up the application in free form. You must indicate the chosen institution for the rehabilitation of the child with a disability. That is, if you choose one of the CSM Clinics, you name us in the submitted documents as the provider of rehabilitation services.

2. Attach copies of the documents specified in the resolution to the application.

When submitting the application, you provide copies of:

• the child's identity document;
• the child's individual taxpayer identification number;
• the passport and taxpayer number of one of the parents or the legal representative;
• the individual rehabilitation program issued by the medical advisory commission (MAC);
• an extract from the medical history, form No. 027/u (no more than one year old);
• a document confirming the child's status as an orphan or a child deprived of parental care (if applicable).

3. After the application is approved, the DLSP will notify the center of the child's referral to an individual correction and rehabilitation program.
4. The medical institution and the DLSP conclude and sign a bilateral agreement. An administrator will let you know and instruct you on the next steps. You will need to fill out questionnaires, consult the rehabilitation course coordinator, and set a session schedule that suits you and the clinic's specialists, and so on.
5. Approximately 5 days before the course starts, the center will inform you by Viber or e-mail of the approximate session schedule.

After that, we meet at the Center and work together toward meaningful progress for your "special" child.

How Children with Disabilities Can Receive Rehabilitation Free of Charge at Our Rehabilitation Center

Today, children with special needs can receive the main rehabilitation services in Kyiv and other cities of Ukraine with state funds. To do this, you need to apply to the relevant authorities.

Rehabilitation Services Children Receive Under the Program

Resolution No. 309 covers the main list of consultation and therapy services that can be provided to a child (including in the rehabilitation of children with cerebral palsy). Among them:

• consultation appointments;
• therapy sessions with specialists in the rehabilitation of children with disabilities (pediatric neurologist, speech therapist, psychologist, pediatrician, rehabilitation specialist, physiotherapist, etc.);
• rehabilitation services (therapeutic exercise, therapeutic massage, physiotherapy procedures, correction of speech disorders, psychological support and correction, pedagogical correction, etc.);
• interim monitoring of the effectiveness of rehabilitation and adjustment of the program (if needed) at each stage;
• instrumental neurodiagnostic methods (when indicated).

Rehabilitation services are prescribed individually. In each case, the personal needs of the child with a disability are taken into account.

The cost of rehabilitation services is determined by the personal program selected for the child. It sets the scope and duration of the treatment, which is funded from the state budget. A patient who comes to the clinic under the program does not pay for the clinic's services.
